What is home care?

Home care in Santa Fe County helps patients make the most of every day of their lives. Managed by Presbyterian Medical Services, a nonprofit organization, Community Home Health Care in Santa Fe provides personalized, expert care for all of our patients.

Home care is supportive care provided in your home by licensed medical professionals and other professional caregivers. Home care helps you avoid unnecessary hospitalization and the expense that comes with it.

Home care is for adult, senior and pediatric patients who are recovering from a hospital or facility stay. Or for patients who need assistance to live safely at home. Patients are more comfortable and less stressed at home which aids in the healing process.

Your home care team.
